 ipate in EUROBAK 20th Bowling Tournament\n\n\n\nDate: 11 October (Saturday

 tion:  Madison Bowling\, Forum Shopping Mall\, 2nd Floor\, 617 Seifullin 
 Avenue\n\n&nbsp\;\n\nTo confirm your participation please\, fill in the Pr
 eliminary Application Form and send it to manager@eurobak.kz no later tha
 n 15 September\, 17.00.\n\n&nbsp\;\n\nBelow you may find the general rules
  of the Tournament:\n\nThe tournament will beheld if not less than 12 team
 s will confirm participation. If not enough teamsregister\, we will have t
 o postpone the Tournament\;\n\nEach team consists of not more than12 peopl
 e (5 players + 7 fans/reserve players). Participation of at least onemembe
 r of the top management team is required\;\n\n&nbsp\;\n\nEach team should:
 \n\n- confirmthat all team members have agreed with the Rules &amp\; Regul
 ations of theTournament (Attachment 1) and send a signed copy to manager@e
 urobak.kz by October 3\;\n\n- submit the Final Entry Form\, signed by all 
 team members and the head of the company (Attachment 2)\nthe original form
  should be provided no later than 3 October\;\n\n- submit a Confirmation L
 etter on the company’s letterhead\, signed by the head of the company (A
 ttachment 3)\nthe original of the form should be provided no later than 3 
 October\;\n\n- send copies of team members’ IDs to manager@eurobak.kz by
  October 3\,originals of ID should be presented to the representatives of 
 EUROBAK Secretariat on the day of the Tournament\;\n\n- one-time membershi
 p fee: 250 000 KZT.\n\n&nbsp\;\n\nNote: during the Tournament participants
